Enrollment Procedures

Steps for Application:
  1. Look for your corresponding section posted on the BULLETIN BOARD.

  2. Proceed to the REGISTRATION SECTION and give the following:
    • For Old Student – Form 138 / Report Card
    • For New Student (G7 and Transferees)
    • For 138 / Report Card – Original and One (1) Xerox copy.
    • Good Moral Certificate
    • Three (3) photocopies of PSA Birth Certificate or any valid equivalent.

  3. Once registered, proceed to the ASSESSMENT SECTION.
  4. Note: There are two (2) Assessment Sections to accommodate the students and parents/guardians.

  5. Once the assessment slip was printed, proceed to the CASHIER’S OFFICE for payment.

  6. Wait for your stamped receipt and check before leaving.
